If you will be attending a program at Hickam, download and fill out this form so we can get you a pass.

IMPORTANT INSTRUCTIONS:

• MUST BE IN BLACK INK!
• If you make a mistake you can’t not scratch it out you must start a new form!
• Provide 2 types of USA identification documents that are not expired.
• Provide the issue and expiration dates of your documents.
• If you are not born in the USA, regardless if you have USA citizenship, you must provide a 3rdtype of USA identification.
• Provide a photo copy of the FRONT and BACK of your photo ID, this ID must be a USA issued photo ID.
  a.      Acceptable forms of photo ID’s are: Real ID (must have gold star/gold star equivalent present) or Passport
  b.      If you are a Green Card and/or Visa holder, you MUST provide a photo copy of each ID
  c.       DO NOT send in a photo of your SSN Card or NON-USA issued ID’s.
• Please make sure to put your weight and height.
• Your signature and initials cannot be typed using a font it MUST be a “wet signature”.
• Leave the Sponsor Name and Sponsor Phone Number Blank

Anyone over the age of 16 will need a form.
